Sciatica is characterised by pain along the course of either of the sciatic nerves. It usually develops acutely; sometimes the appearance of pain is preceded by unpleasant sensations in the leg or lumbar region, the patients complaining that the leg feels heavy, that it tires easily.
Pressure applied along the course of the sciatic nerve produces sharp pain. The following points are usually sensitive: the gluteal fold, popliteal fossa and behind the internal malleolus. Pain along the course of the stretched sciatic nerve-lasegue's sign - is a very constant sign. This sign is tested as follows: the patient is placed supine and the entire extremity is raised with the knee in full extension; as the leg raised the patients feels paint all along its posterior surface. On flex-ion of the leg in the knee the pain along the course of the nerve disappears. Similar paint sensations arise when the patient bends forward (in an attempt to touch the floor with the fingertips) keeping the knees extended.
Sciatica is often marked by a lateral curvature of the spine, which is a reflex posture. In this posture the pain sensations diminish.
Sciatica Causes
The cause of Sciatica is vary. Pathological changes in-vertebral with alteration of their form, which lead to traumatic of the nerve roots emerging through the inter-vertebral foramens, are often observed in cases of stubborn sciatica. Sciatica sometimes caused by infection (influenza, malaria, rheumatism), certain gynecological disease, local cooling and colds, especially on entering a cold room from a hot room.
The cases in which sciatica is caused by inflammation of lumbar and sacral roots are called lumbosacral radiculitis. In these case patienta complain of pain in the small of the back and its irradiation to one leg or both. The pain is intensified on the slightest movement. For example, the patient lying on his back feels greater pain in the small of the back on raising his head, laughing, coughing and sneezing. It is especially painful to change from a horizontal to a vertical position.
Acute sciatica and lumbosacral radiculitis last from 2 to 6 weeks during which there may be remmisions for several days with subsequent exacerbation of the pain syndrome.
In a cases of a longer, protracted course of sciatica it is necessary thoroughly to examine the spine in order to exclude the possibility of tumors in the pelvic organs. The latter cases will show, in addition to the pain symptoms, signs of neuritis - the tendon reflexes will weaken or disappear, the muscles will waste away and the pain sensation will diminish.
